When an AI model generates confident-sounding information that is factually incorrect or entirely fabricated. Hallucinations are a key limitation of large language models and a reason why AI outputs should always be verified for accuracy.

When asked to cite a court case, a legal AI tool invented a plausible-sounding but non-existent case — a hallucination that went unnoticed until the lawyer tried to verify the reference.
